Annual Health and Medical Forms

Medical Forms may be required for participants of certain activities. See the event description or contact your leader for details on which form(s) may be required.

Pack Campouts

EVERY member of the family attending a pack campout MUST have a new medical form on file. Complete parts A and B (No doctor sign-off required). http://www.scouting.org/filestore/HealthSafety/pdf/680-001_AB.pdf

Resident Camp (Overnight)

If your child would like to join Pack 88 please fill out this application and bring it with you to our next pack meeting.

Adult Leader Applications

Pack 88 has active leadership that is dedicated to the character development of each boy. Active parent participation is essential to ensure the success of the Pack. Pack 88 leader positions are listed below.

Adults interested in helping the Pack grow should discuss their interests with the Cubmaster. All adult leaders must complete an application.
